Palm Beach State College Board Policy
Title: Systems Change Control Policy

Purpose
This policy is intended to ensure changes to information technology systems, business processes, information processing facilities and systems that affect information security and the delivery of information services are managed in a rational and predictable manner.
Scope
This policy applies to all Palm Beach State College networks, systems, and services. The policy does not apply to changes made to non-production or development/test systems.
Policy Statement
Every change to a Palm Beach State College production IT resource such as: operating systems, computing hardware, software, networks, and applications is subject to the Change Control Policy and must follow the Change Control Standard Operating Procedures.
A Change Advisory Board (CAB) will meet regularly to review change requests and to ensure that change reviews and communications are being satisfactorily performed.
Palm Beach State College shall establish a scheduled maintenance window. All changes affecting user connectivity, security, and access to IT services must be scheduled within this window of time unless otherwise approved by the CAB.
Normal, standard and emergency changes, as defined in the Change Control Standard Operation Procedures, require the submission of a formal change request prior to the change.
A formal change request is not required to address Break/Fix issues. Break/Fix issues are defined as outages of IT resources that require immediate changes to bring the resource back into working order.
